Disturbed (Nolimit City): Slot Review & Theme
Based on the story, 10% of people have a severe fear of hospitals. This condition is known as Nosocomephobia, and NoLimit City's Disturbed is hardly the right medicine to cure anyone. The game takes place in a horror hospital run by a certain Dr. Death, who views his medical license as a license to kill, per the winning story. It gets worse from there.
Disturbed has a different theme from Mental and is more of a physical horror show than a psychological one. It has Enhancer Cells that provide 5 different modifiers and/or features, and all 4 are always active in the top-tier Annihilation Bonus Round. One is active in the regular Pre-Op feature, and you can win up to 54,391x your stake in all game stages.
Paytable and Bet Size
NoLimit's Disturbed takes place in the demented clinic of Dr. Daniel Eath. A place where, according to NoLimit themselves, top-quality service often leaves patients breathless. Now, it might not be the sort of breathless you'd want going to a medical institution, but for us who aren't actually visiting it physically, it's a breathtaking scene thanks to its incredibly sharp graphics and audio.
Disturbed can be played on all standard mobile devices, including smartphones, tablets, and desktops. A winning combination is created by landing three or more matching symbols on consecutive reels, starting from the leftmost reel.
The lower-paying symbols include various medical equipment. Five-of-a-kind with these pays 2x the bet. The higher-paying symbols consist of blood bags, trash cans, Ms. Happ, Mr. Facelift (not to be confused with Dr. Death), and Dr. Death himself. A winning combination of 5 with one of these pays 2.5x-10x the bet. Plenty of other symbols also activate various mechanics, but more on that in a bit!

Feature of Disturbed (Nolimit City)
Disturbed is a volatile horror hospital slot from NoLimit City, and it plays out on a 4-2-4-2-4 grid with 256 default win ways. You can benefit from 4 Enhancer Cells, and 1 is always active with the xBet turned on and in the regular Pre-op Bonus Round. All 4 cells are always active in the top-tier Annihilation feature, which can award up to 54,391x your stake if you land 5 symbols in a row on reels 1, 3, and 5 during normal gameplay during the free spins round. Our full review is available below the free demo game.
You can turn on the xBet (Ramp Access) ante bet feature in the base game, which will cost you 2x your regular stake to keep it on. With this option switched on, the leftmost Enhancer Cell will always be lit (giving you at least one modifier per spin), and you will also always land a scatter symbol on reel 1. Your bonus round chances increase from 1 in 395 spins to 1 in 143 spins with the xBet switched on.
The premium symbols are Dr. Death and five other characters, who pay 2.5 to 10 times your stake for 5-of-a-kind wins. You can land full-reel character symbols on the middle reel in the base game, and it turns into a full-reel wild in either of the bonus rounds. The wild steps in for all-pay symbols to help complete wins.
The Mrs. Nudge symbol can appear on the middle reel only, and it nudges in place to cover the entire reel. Each step in the nudging process increases the win multiplier by +1. You can also get up to 4 Enhancer Cells active per spin, and active cells light up the reels. This triggers one of the following modifiers per lit cell:
xWays - turns a random position into an xWays position (multiple symbols stacked per position).
xSplit - turns random symbols on the middle reel into Mr. Split wilds.
Wild - turns a random position into a regular wild symbol.
Disturber - awards a respin where activated modifiers retrigger.
Gravestones - 4 gravestones can trigger an Angel of a Death bonus round where you choose one gravestone from 5 options (see below).
The Pre-op Spins Bonus Round triggers from 3, 4, or 5 Scatter symbols anywhere on the screen, giving you 12, 15, or 20 free spins, respectively. You are guaranteed at least 1 Enhancer Cell modifier per free spin in this feature, and the Disturber awards an extra spin. Landing the Executioner scatter upgrades you to the Annihilation Spins Bonus Round with 3 extra spins awarded.
The Annihilation Spins Bonus Round is triggered by 3, 4, or 5 scatters when all Enhancer Cells are lit. You get 12 free spins for 3 scatters, 15 free spins for 4 scatters, and 20 free spins for 5 scatters. All Enhancer Cells remain lit for the duration of the feature, giving you 4 modifiers per spin. The Disturber modifier awards +1 extra spin whenever it triggers.
Angel of Death & Six Feet Under
The Angel of Death and Six Feet Under features are triggered by four lit gravestones, combined with an entire reel of Dr. Death symbols on the middle reel. This takes you to the Six Feet Under max win screen, which cracks open a 54,391x potential prize.
Some jurisdictions offer the Bonus Buy menu, which allows you to purchase special features for your bet. If eligible, you can purchase the Pre-Op feature for 112x your stake (top-tier RTP of 96.12 %) or the Annihilation feature for 666x your stake (top-tier RTP of 96.21 %). You can also pay 250x your stake for a randomly chosen feature (top-tier RTP of 96.25 %).
Disturbed (Nolimit City): Summary
NoLimit City needs more controversial subject matter to sink their teeth into for each new release, as Disturbed comes with a similar theme as Mental. As we've suggested, the unafraid studio would be better served to do regular, lighter themes combined with dark themes like this, as Hacksaw does. When you expect Mental level insanity per release, you grow numb to it, and it's just not as 'disturbing' anymore.
That said, Disturbed is another solid release from the developer that doesn't break any new ground but offers fun nonetheless. The betting feature is excellent because it allows you to shorten the time between bonus rounds while giving you a modifier per spin. The volatility is high as always, with a 54,391x potential coming with a 1 in 10m spins hit rate from the base game, a 1 in 57K spins hit rate from the Pre-Op feature, and a 1 in 1.5K spins hit rate from the Annihilation feature. The latter is hard to trigger, so this isn't for people with Nosocomephobia.
Dare to enter the demented clinic of Dr. Daniel Eath, where top-quality service often leaves patients breathless... Behind the curtains of a seemingly ordinary office, the Doctor will see you now.
